Seeking a full-time, energetic, professional Admissions Case Manager based in Key West who will interact with healthcare providers throughout the Lower Keys. This physician liaison will work directly with the Intake & Referral Specialist, clinical staff, community medical facilities,

physicians, and physicians' staff.

Successful candidate will have excellent communications skills (written & spoken); as well as the ability to work with physicians, discharge planners, and other healthcare providers.

A background in healthcare is preferred.

This per-diem position is responsible for assessing patients utilizine history and physical assessments; develop plans of care, as well as provide nursing interventions including direct care and patient education. RN is responsible for colloborating and communicating with team members, as well as documenting patient data.

Job Requirements: strong cummincation skills, knowledge of computers and Electronic Medical Records, and ability to work independently. Current license to practice in Florida, valid Florida's driver license, and reliable transportation. Minimum of one year experience in medical/surgical, home health or hospice nursing preferred.

PHYSICAL THERAPIST / OCCUPATIONAL THERAPIST

Full and Part-time Positions Available Now!

The qualified candidate will be responsible for assessing, planning, implementing and evaluating therapy programs in order to restore function and prevent disability in home-health patients. Consults with and educates the patient, and family, regarding the functional implications of the disease process, self-care techniques, and prevention strategies. Works in collaboration with patient's physician and interdisciplinary team members. Documents patient data in an accurate, concise and complete fashion.

Job Qualifications: Minimum of two years experience, prefer in-home; current license to practice in Florida; valid Florida's driver's license and reliable transportation; current CPR certification and state-mandated education requirements; prior experience with Electronic Medical Records (EMR).
